Why Supported Living Pathways?

We have an excellent track record and more than 12 years’ of experience in providing and managing recovery pathways for clients with multi -diagnosis and very complex needs. We pride ourselves in our 95% plus success rate in transitioning clients from Mental Health Hospital to independence back in their own homes within the community with ongoing support as appropriate.

Mental Health Professionals including Responsible Clinicians, Commissioners, Specialist Placement and Brokerage teams, Care Managers and Social Work Practitioners all entrust us with the responsibilities of taking good care and achieving most positive outcomes for their clients.

Pathways and Care Packages

Our “Sea Breeze” project offers High-Level Support with 24/7 staff on site. The service comprises a 4 bed female unit, 3 bed male unit and 2 independent bedsits. We also provide a further 6 bedsits nearby which serve as an initial moving on pathway towards greater independence with lower support levels. We also have a further 8 bedsit project in the pipeline, subject to obtaining the appropriate planning consents.

 

From experience this works well in practice for our clients. Moving on to lower support creates increased self-confidence and promotes feelings of achievement and progress for service users without being overwhelmed with sudden changes or a situation where they struggle to cope.

 

Our Care Packages present excellent value for money and enable substantial savings for the NHS and Social Services whilst relieving pressures on capacity in overstretched Mental Health Hospitals.

Safeguarding

We prioritise managing ongoing risks and protecting each individual service user`s wellbeing, health, human rights and best interests. These principles help us to provide them a home where they are able to live free from harm, abuse and neglect.

Our robust safeguarding policies, procedures and ongoing staff training enable us to identify, prevent and respond to potential harm or abuse. We aim to empower our staff team to recognise and act upon any safeguarding concerns.

Referral Process

Supported Living Pathways is a Registered Adult Social Care Provider with Sussex, Brighton & Hove and multiple other London Local Authorities and NHS Foundation Trusts including North West London, Hillingdon and Newham. We have been able to provide positive and successful outcomes for some of their most challenging and complex cases.

We recognise the ever changing market needs and now also accept Spot Purchase placements where out of area placement may be the most beneficial and appropriate solution.

Our experienced team will assess potential client`s individual needs and work closely with Procurement and Brokerage Teams, Care Managers, Mental Health Tribunals and other agencies to achieve the appropriate CPA and discharge pathway for each service user`s individual circumstances.
